Abstract

Context: Food allergy plays a significant pathogenic role among children with atopic dermatitis (AD).

Objective: The aims of this study were to evaluate allergy to egg in these children and determine the egg specific immunoglobulin E (IgE) cutoff point.

Design and Setting: It was a cross-sectional study that took place at Immunology, Asthma and Allergy Research Institute from 2005 to 2007.

Methods: Children younger than 14 years old with AD entered the study. Careful medical histories were taken and skin prick and Immuno-CAP tests with the most commonly offending foods (cow’s milk, egg, wheat, peanut, and soy) were performed. Children with a clear, positive history of food allergy and a positive IgE-mediated test or those with positive responses to both IgE-mediated tests were determined to have food allergies. The egg-specific IgE level cutoff point was determined.

Results: A hundred patients entered the study (from 2 months to 12 years old). They were divided into 3 age groups: first <2 years, second from 2 to <6 years, and third from 6 to 14 years. The most common food allergens were egg (39.22%) in the first, cow’s milk (35.13%) and egg (32.43%) in the second, and peanut (25%) and egg (16.67%) in the third group. The egg-specific IgE cutoff point value was 0.62 kUA/L (kilounits of allergen-specific IgE per liter). The positive predictive value was 95%.

Conclusion: Prevalence of egg allergy is highly significant in patients with AD.To use egg-specific IgE level cutoff point, the patient population under study must be considered.

The hypothesis that certain foods or allergens might bring about convulsions has been suggested repeatedly in the literature over the last century. Some clinical studies have highlighted an unusually high prevalence of allergic disorders in patients with epilepsy. This paper reports the consistent disappearance of partial idiopathic epilepsy symptoms in a nine-year-old patient as a result of diet free of cow’s milk protein. This case appears to confirm the possible role of food allergy in certain types of epilepsy in patients of pediatric age.

OBJECTIVES: The possibility that certain foods or allergens may induce convulsions has already been reported in the literature. None of the relevant studies has, however, shown a close correlation between allergy and epilepsy, most reports being anecdotal and open to various causal hypotheses. The case-control study reported here was undertaken to test the hypothesis that epilepsy is linked to allergy. MATERIAL AND METHODS: Seventy-two epileptic children and a group of 202 controls in the same age bracket were investigated for allergy together with their immediate families. RESULTS: The study revealed significantly higher rates of eczema in the mothers and rhinitis in the siblings of the patients studied as well as generally higher incidence of allergic pathologies in both of these groups with respect to the relevant controls. A significantly higher incidence of allergy to cow’s milk and asthma was also documented in the epileptic children with respect to the control group. Prick tests gave a significantly higher rate of positive results for cow’s milk proteins in the cases examined with respect to the controls. The total serum IgE of a random sample of cases and controls showed no difference in mean values. CONCLUSION: The study appears to bear out the hypothesis of a higher incidence of allergy in the children with epilepsy and their immediate families than in the controls and their families.

The possibility that certain foods or allergens may induce convulsions has already been reported in the literature. None of these studies has, however, shown a close correlation between food allergy and epilepsy, most reports being anecdotal and open to various aetiological hypotheses. The present report concerns 3 children with cryptogenetic partial epilepsy, diagnosed by means of electroencephalography, with behavioural disorders (hyperactivity, sleep disorders and writing difficulties). In these patients, instead of using anticonvulsive agents, treatment was based upon a cows milk-free diet, working on the hypothesis that there could be a casual relationship between intolerance to this food item and the epileptic symptoms. An improvement was observed in the children’s behaviour and moreover, the electroencephalographic anomalies disappeared. Upon double blind oral provocation tests, these patients did not present an immediate reaction, but only after a few days. starting the controlled diet again led, in all cases, to disappearance of the electroencephalogram. In conclusion, it would appear feasible to hypothesize the role of food intolerance in the onset of convulsive crises, even if limited to certain types of epilepsy such as the cryptogenetic partial form.

Further to your main article on epilepsy (WDDTY vol 14 no 4), a series of studies was carried out at the Great Ormond Street Hospital in the 1980s under Professor Soothill, with Jo Egger and Christine Carter. They found evidence of epilepsy being caused by allergy when there was other evidence of allergy. Certainly, I can think of two children who remained free from epilepsy by paying attention to their allergies. The study was published in the American Journal of Paediatrics (1989; 114: 51-8), entitled ‘Oligoantigenic diet treatment of children with epilepsy and migraine’.

A large body of opinon believes food allergy underlies the para-epilepsies-narcolepsy and cataplexy-disorders with sudden sleep attacks and sudden muscle weakness and control, respectively, but not epilepsy proper. Nevertheless, there is considerable

This has been long known by naturopaths and been the subject of numerous popular books, but a number of double-blind, placebo-controlled trials have also shown a connection.

In the UK, Dr Jonathan Brostoff describes a most comprehensive trial involving 88 children (40 of whom underwent a placebo-controlled double-blind trial) in 1982 and 1983 at Great Ormond Street Hospital for Sick Children. All these children suffered from migraine and a proportion of them also had recurrent epileptic fits. Once their allergies had been identified and the offending foods removed, 78 recovered completely from their migraine attacks and another four improved greatly. But most importantly, their epilepsy disappeared as well (J Brostoff and L Gamlin, The Complete Guide to Food Allergy and

Intolerance, Bloomsbury Publishing, Ltd, 1989).

Brostoff also provides evidence demonstrating that exposure to certain chemicals can precipitate allergic epileptic seizures. These include gases, such as industrial air pollutants, airborne droplets (such as aerosols and pesticide sprays), smoke (coal smoke, or fumes from paraffin burning stoves) and other combustion products (such as two-stroke engine exhaust fumes or diesel emissions), solvents (varnish or paint) and either volatile (mothballs and aftershave lotions) or non-volatile (drugs or food additives) compounds.

However, the most compelling evidence concerns the relationship between celiac disease (allergy to foods containing gluten wheat, oats, barley and rye) and epilepsy.

In one study of children comparing one group following a glutenfree diet and the rest with restricted or unrestricted gluten intake, epilepsy only occurred in children given an unrestricted diet (Arch Dis Child, 1994; 70 (3): 211-3). Other studies have demonstrated an association between brain calcifications and celiac disease (Child Nerv System, 1993; 9 (3): 172-5).

Other evidence demonstrates that patients with low calcium levels due to malabsorption syndrome are at particularly increased risk (Harefuah, 1994; 126 (12): 707-10, 763).

One of the largest studies to date was conducted in Italy, by the Italian Working Group on Celiac Disease and Epilepsy. They examined 43 patients with cerebral calcifications and epilepsy by having them undergo an intestinal biopsy or CT scan. Twenty-nine (or 77 per cent) showed evidence of the characteristics of celiac disease, the atrophy of the villi, or tiny hairs of the gut, or brain calcifications (The Lancet, 1992: 340: 439-43). In the view of most doctors, a gluten-free diet will help the epilepsy only when embarked upon soon after the epilepsy began a view that many allergists don’t share.

Besides food and chemicals, emotional upsets also commonly trigger seizures. Consequently autogenic training, mental imagery, meditation or biofeedback have all been shown to reduce the frequency and severity of seizures.

Allergies may be more than just skin problems. An association between allergic and migraine headaches  has been determined. People with allergic rhinitis appear to be 14 times more likely to get migraines compared to someone without nasal allergies.

Migraine headache is a chronic condition affecting approximately 6% of men and 18% of women. Symptoms can be severe, disrupt daily activities, require bed rest, and potentially last for days. The cause of migraines is not completely understood, although is thought to be related to chemicals in the body that cause blood vessels in the brain to dilate, which can lead to the headaches.

A recent study of children and adults with nasal allergies found that there may be a link between allergies and migraines. Histamine, the compound that causes allergy problems may also help trigger migraines. During an allergic reaction, your immune system releases histamines.

The study looked at nearly three hundred people — both children and adults — and the presence or lack of migraine symptoms. For people with allergic rhinitis (also known as hay fever), a third of the participants met the diagnostic criteria for migraines. For study participants without allergy issues, only four percent of participants met the diagnostic criteria for migraines. When the study results were tallied, it seemed that patients with allergies were fourteen times more likely to have symptoms of a migraine than patients without allergies

This association is due, at least in part, to the release of histamine from allergic cells in the body, which makes blood vessels dilate, or relax. The same process can occur in blood vessels in the brain, possibly leading to a migraine headache.

The migraines occurring in allergy sufferers might respond to typical elimination diet,  treatments for allergic rhinitis (such as anti-histamines and prescription nasal sprays), as well as to allergy shots.
